Outreach Services

Many seniors live alone, without nearby family or a support system. Some are physically limited, others struggle financially, and many simply don’t know what resources are available to help them age in place safely and with dignity. Our outreach programs are designed to close that gap—ensuring no one falls through the cracks.

By identifying seniors who may not seek help on their own, we prevent small issues from becoming serious crises. Early intervention allows us to address unmet needs, provide safety checks, and improve overall well-being.

Our outreach services are made stronger through collaboration. We partner with local health departments, senior centers, housing authorities, and social service agencies throughout the county to provide comprehensive support. Together, we ensure that each senior receives the right care at the right time.

Helping Older Adults Access the Resources They Deserve

Navigating the maze of government benefits, community programs, and support services can be overwhelming for anyone—especially for seniors who may not know where to begin. That’s where Information and Referral (I&R) Services come in. These services connect older adults with the right programs, resources, and assistance to help them live safer, healthier, and more independent lives.

From utility discounts and legal advice to mobility aid loans and prescription coverage, I&R Services ensure that seniors don’t miss out on the support they’re entitled to. These programs are typically available through local Councils on Aging, senior centers, or Area Agencies on Aging, offering both walk-in help and phone consultations.

Information and Referral Services are designed to provide one-stop access to comprehensive community resources for seniors aged 60 and over. Specialists maintain extensive resource files covering hundreds of local, state, and federal programs. Whether you’re looking for financial help, medical support, or housing services, I&R professionals can guide you in the right direction—and often help with applications, too.

  • Program eligibility screening
  • Application form assistance
  • Referrals to trusted local providers
  • Help understanding benefit letters or paperwork
  • Follow-up support for time-sensitive needs

These services are offered free of charge and are tailored to each individual’s specific needs, including low-income seniors, those with disabilities, or caregivers seeking help on behalf of a loved one.

Many seniors live on fixed incomes and struggle with rising utility bills or essential living costs. I&R Services provide information and help with enrollment in several discount programs:

This program provides one-time financial assistance to help low-income seniors pay winter heating or summer cooling bills. In many areas, emergency HEAP is also available for those facing disconnection.

A federal program that offers discounts on telephone or internet service, helping seniors stay connected to family and emergency services.

Free energy-efficiency improvements such as insulation, window sealing, or furnace repair for qualifying low-income homeowners. This helps reduce long-term utility costs.

Health coverage can be complicated, especially for those navigating Medicare, Medicaid, and private insurance. I&R specialists can help explain options and assist with paperwork for a variety of health-related programs:

Assistance understanding and enrolling in the Medicare Part D prescription program, including help comparing drug plans.

Guidance on filling out claim forms and appeals for Medicare or private insurance, and help understanding benefit statements.

I&R staff may refer seniors to local case managers or care coordinators who help organize services like in-home care, transportation, and meal delivery.

Older adults often face unexpected expenses—whether it’s a medical emergency or help affording basic home modifications. I&R Services provide referrals to programs that can help.

Also known as “Extra Help,” this federal program reduces prescription drug costs for low-income Medicare beneficiaries.

A benefit program for older adults with limited income and resources. I&R specialists can assist with applications or referrals to Social Security offices.

Access to free or low-cost legal assistance for seniors, including issues related to housing, benefits, or elder abuse.

Help preparing advance directives that allow seniors to make decisions about medical care and finances, even if they become incapacitated.

I&R staff can help seniors register to vote or request absentee ballots, ensuring they remain active participants in civic life.

Many older adults need help obtaining medical equipment but can’t afford to purchase it new. I&R Services provide information about equipment loan programs in your area.

Temporary or long-term access to mobility aids like:

  • Wheelchairs
  • Walkers
  • Canes

This helps reduce fall risk, increase safety at home, and improve quality of life without the high cost of medical supply stores.

Exclusive to Ohio residents, the Golden Buckeye Card offers discounts at thousands of participating businesses across the state, including:

  • Prescription drug savings
  • Grocery and retail discounts
  • Restaurant and service coupons

Most Ohioans automatically receive the card when they turn 60, but I&R staff can assist with replacements or questions about using the card effectively.

Getting help is as easy as calling your local senior center or Council on Aging. Many also offer drop-in assistance, scheduled consultations, or outreach visits for those unable to travel.

Some agencies also have printed resource guides or online directories to help you explore services from home. I&R professionals are specially trained to provide confidential, respectful support—no question is too small.

Information and Referral Services are often the first step for older adults who feel overwhelmed by paperwork, benefits programs, or unexpected life changes. Whether you’re newly retired, living with a chronic condition, or supporting an aging parent, these services help simplify the path to health, independence, and security.

From legal help and financial relief to mobility aids and wellness programs, the resources are out there—I&R Services help you find them.

Aging should never mean facing challenges alone. With Information and Referral Services, seniors can tap into the wealth of programs designed just for them. Whether it’s accessing HEAP to stay warm in winter, enrolling in Medicare drug coverage, preparing a living will, or borrowing a walker—help is available.

Don’t hesitate to reach out. A simple phone call could connect you to the support that makes all the difference.
